Change of Ownership of a PLDT Telephone Line (Deed of Assignment)

DEED OF ASSIGNMENT AND TRANSFER OF RIGHTS

(CHANGE OF OWNERSHIP)

KNOW ALL MEN BY THIS PRESENTS:

This deed, made and entered into this 10th day of January, 2016 at the City of Manila, by and between:

MARITES LAPIRA SOLIVEN, Filipino Citizen, of legal age, married to EMELINO TOLENTINO MAESTRO with residence and postal address at Unit 419 Corporate 101 Mother Ignacia Quezon City, hereinafter referred to as the “ASSIGNOR" -and-

EMELINO TOLENTINO MAESTRO, Filipino Citizen, of legal age, married to MARITES LAPIRA SOLIVEN with residence and postal address at No. 88 V. Luciano, Barangay Masagana, Project 4, Quezon City, hereinafter referred to as the "ASSIGNEE".

WITNESSETH that -

WHEREAS the ASSIGNOR is the true and sole owner of a PLDT landline numbered 02-521 7776 as it is evidenced by a PlDT Customer Information Sheet No. RB 6862655 dated December 21, 2015 which marked as Annex 1 and forms part hereof.;

WHEREAS, the ASSIGNEE has offered to buy the ASSIGNOR’s rights, title and interest over the above phone and the ASSIGNOR hereby accepts the ASSIGNEE’s offer;

NOW, THEREFORE, for and in consideration of the foregoing premises and the sum of ONE PESO (1.00), PHILIPPINE CURRENCY, which the ASSIGNOR hereby acknowledged to have received from the ASSIGNEE, the ASSIGNOR hereby assigns, transfers and conveys unto the ASSIGNEE, all her rights, title and interest to the aforementioned telephone landline pursuant to this Agreement and the ASSIGNEE by these presents hereby accepts the assignment and agrees to be bound by the terms and conditions, rules and regulations, and restrictions pertaining thereto.

IN WITNESS WHEREOF, the parties have hereunto set their hands on the date and place first above written.
